ASC LeadER - Confirmation of Completion and Evaluation

Welcome to the ASC LeadER Confirmation of Completion and Evaluation eForm, which enables you to confirm the completion of a learning activity or online course and provide feedback. The Social Care Academy uses the information you provide to monitor the level of learning completed, review feedback and produce a Certificate of Completion for those employer by a Care Sector Provider.

The information provided on the Form will be processed in line with the General Data Protection Regulations (GDPR) / Data Protection Act 2018. Further information can be found on www.tinyurl.com/erycpublicnotice.
